You are responsible for the payment of session fees.
After payment, I can provide an invoice (Superbill) for you to request reimbursement from your insurance company.
Credit Card Information is required at booking.
If we decide not to move forward with treatment in the first 15 minutes of your session, you will not be charged.

Step 2: Request an appointment time.

Step 3: Receive confirmation of appointment time.
Within 48 hours of your request, you will receive an email confirming your appointment time. You will also receive access to your patient portal.
Step 4: Complete your paperwork.
Complete the required forms in your patient portal at least 24 hours before your appointment.
Step 5: Meet Me!
Finally, it’s time to get to know each other! :D
The first 15 minute consultation will be screening to ensure that we are a good fit. You’ll have the opportunity to ask questions and decide if you would like to move forward with the appointment. If we decide in the first 15 minutes that it’s not a good fit, no worries! I can refer you to another provider and you won’t be charged for the session.
